1) Chlorine residual measured at time of inspection.|2) Facility has added an ice machine in back hallway behind bar.|3) Facility has added a pizza oven to back hallway behind bar to cook frozen pizzas.|4) Facility planning a 8/30 Clam Bake. Temporary Food Service Permit application supplied at time of inspection, and previously emailed to owner. Molluscan Shellfish guidance distributed at time of inspection. DOH will need completed: Application, sanitization plan, and fee. DOH already has workers compensation and disability forms on file for facility. If other events are known operator encouraged to add on to one application to ensure proper processing.
