Cold holding temperatures on the buffet line and all of the cooler units in the kitchen were below 45 degrees F|The waiver log for the Sushi rice was being maintained |overall the facility is in poor repair and many of the surfaces are extremely dirty and in poor condition |The facility need to have a through cleaning and a lot of maintenance done|The facility has added some new items to the buffet and are struggling to keep those items hot as well. The staff are not completing the requirements needed for the waiver to work on the buffet line. |Did not see any evidence of cockroaches at time of this inspection|Operator is still having pest control come one time per month
