School-Based Health Center Enrollment Moves Online

North Country Family Health Center is moving enrollment for its school-based health centers to a digital process, according to information from South Jefferson Central School District.

Families enrolling a child who has never used the centers can complete a questionnaire and will then receive a text message with a secure link to enrollment forms. Families whose children are already enrolled will receive a text with a link for annual paperwork. Paper forms will remain available upon request and on the center’s website.
